bulletPolio — 3 doses with one after 4th birthday
bulletDTP — 4 doses with one received after 4th birthday
bulletMMR — 2 doses or 1 MMR and 1 measles
bulletHep B — 3 doses
bulletVaricella — 1 dose or signed statement from parent or doctor stating that the child had (Chickenpox) chickenpox and the date
